What is C.H.A.P.?

C.H.A.P. stands for Chaplain Helpful Assistance Program.

It's a free service that helps connect you with community resources like food assistance, counseling, recovery meetings, and more.

A chaplain, social worker, or case manager used this tool to find services that fit YOUR schedule and location.

Access Your Care Plan Online

You can view your care plan anytime through our secure Client Portal. This is perfect if you lost your printed copy or need to look up service details.

How to Access Your Care Plan

  2. 2Enter your email address or phone number
  3. 3Check your email or text messages for a verification code
  4. 4Enter the code to securely access your care plan

Note: The verification code expires after 10 minutes for your security. If you don't receive a code, check your spam folder or contact your care coordinator.

Understanding Your Care Plan

You may have received a text message with your care plan, or been given a printed copy. Here's what it includes:

Your Matched Services

Each service in your care plan includes:

  • Service Name - The organization or program
  • Type - What kind of help they provide (food, counseling, etc.)
  • Recommended Day & Time - When to visit based on your schedule
  • Address - Where the service is located
  • Distance - How far from your location

  • Access expires automatically - You can set how long your authorization lasts
  • You can revoke access anytime - Contact your care coordinator to change who can see your data
