These Terms of Service ("Terms") govern your use of the CHAP (Chaplain Helpful Assistance Program) service provided by Midwest Cyber, LLC ("we," "us," or "our") at https://chap.midwestcyber.net (the "Service").
By accessing or using the Service, you agree to be bound by these Terms. If you do not agree to these Terms, please do not use the Service.
Service Description
CHAP is a care coordination platform that helps connect individuals with community services and resources. The Service enables:
- Care coordinators to create and manage client assistance plans
- Matching clients with appropriate community services based on their needs
- SMS notifications to keep clients informed about their care plan
- Secure storage and management of client information
SMS Messaging Terms
By opting in to receive SMS messages from CHAP, you agree to the following:
Consent and Opt-In:
- You consent to receive SMS notifications regarding your assistance plan
- Consent is not a condition of receiving services
- Message and data rates may apply
- Message frequency varies based on your care plan activity
Opt-Out:
- Reply STOP to any message to opt-out of SMS communications
- Reply HELP to any message for assistance
- You may opt back in at any time by replying START
Message Types:
- Care plan notifications and updates
- Service appointment reminders
- Verification codes for portal access
- Responses to your inquiries (HELP, MAP commands)
Carriers are not liable for delayed or undelivered messages. For support, reply HELP to any message or call 211.
User Responsibilities
As a user of the Service, you agree to:
- Provide accurate and complete information
- Keep your contact information up to date
- Notify us if your phone number changes
- Use the Service only for lawful purposes
- Not attempt to access accounts or information belonging to others
- Not interfere with or disrupt the Service
Care Coordinator Terms
If you are a care coordinator using the Service, you additionally agree to:
- Obtain proper consent before entering client information
- Comply with all applicable HIPAA requirements
- Access client information only when necessary for legitimate care purposes
- Protect your login credentials and not share them with others
- Report any suspected security incidents immediately
- Complete required training as assigned

THE SERVICE IS PROVIDED "AS IS" AND "AS AVAILABLE" WITHOUT WARRANTIES OF ANY KIND, EITHER EXPRESS OR IMPLIED.
- CHAP is a care coordination tool and does not provide medical advice, diagnosis, or treatment
- We do not guarantee the availability, quality, or suitability of any community services listed in the platform
- For emergencies, call 911. For urgent assistance, call 211.
Limitation of Liability
TO THE MAXIMUM EXTENT PERMITTED BY LAW, MIDWEST CYBER, LLC SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING OUT OF OR RELATED TO YOUR USE OF THE SERVICE.
Privacy
Your use of the Service is also governed by our Privacy Policy, which describes how we collect, use, and share your information.
Changes to These Terms
We may update these Terms from time to time. We will notify you of any material changes by posting the new Terms on this page and updating the "Last Updated" date. Your continued use of the Service after any changes constitutes acceptance of the updated Terms.
